Azura’s assisted living and memory care community in Brookfield at Mierow Farm features four homes on its grounds; two homes (Elmbrook and Mierow) specializing in memory care for those with early to late stage dementia and Alzheimer’s disease, and the other two homes (Broadway and Wirth) for those seeking general assisted living. The picturesque community features a historic barn owned by the Mierow family and seasonal farm stand that has been preserved from its previous operation, as well as a community garden for all to enjoy!

Our Elmbrook and Mierow memory care homes are designed to provide a holistic approach that supports and empowers residents to live a joy filled life. With over 120 hours of dementia specific training within their first 90 days, our team members are taught how to provide support while maintaining dignity to those with memory loss.

Our Wirth and Broadway assisted living homes are designed to offer personalized assistance to support independence and well-being. With convenient on-site amenities, chef-prepared meals, social activities and more, our residents enjoy life in a vibrant community full of possibilities.

Our Elmbrook and Mierow homes provide specialized memory care for residents living with early- to late-stage Alzheimer’s disease and other forms of dementia. Both homes follow Azura’s signature MOSAIC philosophy – a person-centered approach that honors each resident’s life story, preferences, and abilities to create meaningful daily moments rather than a one-size-fits-all routine.

Every team member who supports memory care residents at Brookfield completes more than 120 hours of dementia-specific training within their first year of employment. That training covers communication techniques, behavior support, sensory engagement, and end-of-life care, so families can trust that the people caring for their loved one understand the disease and the person living with it.

Caregiver-to-resident ratios are intentionally kept low so residents receive consistent, unhurried support. Daily programming is built around the MOSAIC framework and includes music, art, reminiscence activities, light exercise, sensory experiences, and structured social connection – programming designed to reduce anxiety, support cognition, and bring joy.

Both Elmbrook and Mierow are secured neighborhoods with monitored entrances and exits, enclosed outdoor courtyards, and safety features built into every detail of the home – giving residents the freedom to move and explore safely while giving families peace of mind.

Rates at Azura Brookfield range from $5,892 to $12,148 per month, based on the level of care a resident needs and the room or floor plan selected. The all-inclusive monthly rate covers private accommodations, three chef-prepared meals daily plus snacks, housekeeping and laundry, utilities, the full activity calendar, 24-hour team member support, and personalized care services.

Two main factors drive where a resident’s rate falls within that range:

  • Level of care. Assisted living residents who need minimal daily support pay less than residents who require extensive personal care or memory care services.
  • Room and floor plan choice. Studio, one-bedroom, and two-bedroom floor plans (ranging from approximately 280 to 719 square feet) are priced accordingly.

For families weighing the cost of senior living, it helps to compare it to the alternative. Around-the-clock private in-home care typically costs $23,000 or more per month in southeastern Wisconsin, and that figure does not include meals, housing, utilities, social engagement, or specialized dementia programming – all of which are built into the Azura monthly rate.

A note on payment sources: Azura Brookfield is a private-pay community and does not accept Medicaid upon admission. We do our best to work with families after private pay funds have been exhausted, after two to three years.  Long-term care insurance will reimburse families for care, and we work with veterans and surviving spouses who qualify for the VA Aid & Attendance benefit, which can offset a meaningful portion of monthly costs. Our team is happy to walk through funding options during your tour.

Every team member is grounded in our MOSAIC philosophy and supported by Azura’s nationally recognized education program – including 90+ hours of dementia training in the first 90 days for those serving in our memory care homes. On-site clinical and therapy services

As care needs evolve, Azura Brookfield brings the right specialists directly to residents – so families don’t have to coordinate appointments, transportation, and unfamiliar offices. Through close collaboration with home health and hospice providers and area physicians, residents have access to:

  • Physical, occupational, and speech/swallow therapy to support recovery, mobility, and communication
  • On-site physician visits for routine and ongoing medical care
  • On-site dental and podiatry services
  • Wound care and skilled nursing support through our home health partners
